There are two types of hemorrhoids (often also called “piles“):


Internal hemorrhoids are swollen veins inside the rectum that form in an area where there aren’t any nerve endings. When internal hemorrhoids become enlarged, the tissue may protrude out through the anus causing symptoms such as itching, bleeding, and swelling. The level of protrusion indicates the degree of severity.


External hemorrhoids often occur as bulges or lumps around the anus. Because of the sensitive nerves in this area, these abnormal vessels are often painful, especially when sitting. If an external hemorrhoid becomes thrombosed, it may even cause severe pain.

Hemorrhoids are graded on a scale from I to IV.

Piles is classified into four grades:

  • Grade I: There are small inflammations, usually inside the lining of the anus. They are not visible.
  • Grade II: Grade II piles are larger than grade I piles, but also remain inside the anus. They may get pushed out during the passing of stool, but they will return unaided.
  • Grade III: These are also known as prolapsed hemorrhoids, and appear outside the anus. The individual may feel them hanging from the rectum, but they can be easily re-inserted.
  • Grade IV: These cannot be pushed back in and need treatment. They are large and remain outside of the anus.


External piles form small lumps on the outside edge of the anus. They are very itchy and can become painful if a blood clot develops, as the blood clot can block the flow of blood. Thrombosed external piles, or hemorrhoids that have clotted, require immediate medical treatment.

Recovery

Once your doctor’s visit is complete, that’s it – no restrictive hemorrhoid recovery regimens to follow. You can resume your normal activities as tolerated. Simply avoid heavy lifting, rigorous exercise and similarly strenuous activity on the day of your treatment. You can resume all normal activity the next day.


You can have normal bowel movements during hemorrhoid recovery, though you may want to soak in a sitz bath (a warm tub with a tablespoon of table salt added) or use a bidet for a gentler cleansing of the anal opening.


Preventing future hemorrhoids

After recovery, you’ll need to make some changes to help prevent future problems. To prevent hemorrhoids, you’ll need to avoid straining during bowel movements. Some tips include:


  • Drink a lot of fluids. Unless your medical condition prohibits it, you should consume seven or eight glasses of water each day.
  • Add 15 grams of fiber to your diet (two tablespoons of natural oat or wheat bran, Metamucil, Benefiber, flax or other soluble fiber).
  • Do not sit longer than two minutes on the toilet. If you can’t have a bowel movement in that time, come back later. This “two-minute” rule can help keep you from straining during bowel movements without realizing it.
  • During air travel, be sure to stay hydrated, avoid alcohol, eat fiber and walk around when you can.

Creams and Home Remedies – Hemorrhoid creams can be good in providing temporary relief during a flare-up, however, they just temporarily alleviate discomfort like burning, itching and inflammation without getting to the root of the problem.


Hemorrhoidectomy – As mentioned, a hemorrhoidectomy is a surgical procedure to remove hemorrhoids. Typically a hemorrhoidectomy is only needed for the most severe cases. Types of surgical treatments include:


  • Conventional surgical hemorrhoidectomy
  • Stapled hemorrhoidectomy (PPH)
  • Doppler-guided Ligation of Hemorrhoidal Arteries (THD)

With any type of surgery there are severe risk factors involved which is why the CRH O’Regan System, non surgical approach is highly sought after by patients.


Infrared Coagulation (IRC) This is a procedure that uses hot infrared light to treat small or mid-sized hemorrhoids. As the treatment tends to be more superficial, often times multiple procedures are required and the wait period between treatments is usually 90 days.


Ultroid Non-Surgical Hemorrhoid Treatment This is a procedure where a probe is introduced into the anus through an anoscope which uses dc electrical current (“direct current”) to cause the hemorrhoid to scar and shrink.


Sclerotherapy: Chemical Injection Treatment This treatment involves injecting a chemical through an anoscope into the hemorrhoid tissue, causing it to scar and shrink. Sclerotherapy is rarely practiced today because it has a high potential for complications compared to more safe and effective procedures, like rubber band ligation.
